It is the purpose of Burks Supportive Living to provide an array of services designed to create appropriate options to individuals with intellectual disabilities/developmental disabilities as defined by the Tennessee Department of Intellectual and Developmental Disabilities (DIDD).
Burks Supportive Living as been approved by (DIDD) to provide services to individuals through the Home and Community Based Services (HCBS) waivers within the East Tennessee region and has an active “Provider Agreement” in place
Funding for these services comes from DIDD Home and Community Based Service Waivers and shall include: Supported Living, Intermittent Employment and Community Integration Wrap-Around Supports (Not Home Bound), Non-Residential Homebound Support Services, Community Participation Supports, Supported Employment. Burks Supportive Living office location hours of operation are Monday through Friday from 9am to 4pm EST
⦁ Before admittance to the Burks Supportive Living programs, all referrals should be certified for placement and funding through the Home and Community Based waiver programs.
⦁ After receiving the referral information an interview will be held with the potential person served. Those involved in the interview process will meet to discuss the applicant’s abilities and limitations.
⦁ The final decision on placement will be based on the referral’s ability to live independently with supports, the desire to live in the community, the need for placement, fitting the established criteria for admission, and the availability of funds.
⦁ Applicants shall be screened by Business Manager and Program Coordinator/House Manager prior to admission
⦁ Burks Supportive Living shall review the “Individuals Support Plan” ISP to make sure their plan of care aligns with BSL program elements.
⦁ Must be aged 18 or older.
⦁ Applicants shall be an individual with a developmental disability, with the disability being attributable to intellectual disabilities, cerebral palsy, epilepsy, or other neurological conditions which are closely related to intellectual disabilities.
⦁ There shall be no restrictions on the person served due to race, creed, color, age, religion, gender, sex, sexual orientation, gender identity or expression, national origin, veteran status, disability, or gender information.
⦁ Individual decisions regarding placement or continual placement are made by the Program Coordinator/House Manager and the Executive Director with input by Individual Support Plan (ISP) team members.
1. Any discharge decision and plan will be established by a team of Burks Supportive Living (BSL) staff members, including Case Management (CM), with the participation of the person receiving services and/or their legal guardians or representatives.
2. A discharge summary, containing the reason for discharge, the diagnosis, the functional limitations, the services provided, the results of services, and any referral action recommended will be completed by the Program Coordinator/House Manager and entered into the case record of the person served.
